
Perineal urethrostomy is a surgical procedure performed primarily in male cats to create a new,
wider opening in the urethra at the perineal area. It’s most commonly indicated to treat recurrent
or obstructive feline lower urinary tract disease (FLUTD), especially in cases of urethral
obstruction due to stones, strictures, or swelling.
Indications
– Recurrent urethral blockage (obstructive idiopathic FLUTD).This is common in male cats as
compared to female cats.
– Urethral strictures or narrowing
– Urethral trauma or trauma-related strictures
– Urethral stones or debris not amenable to less invasive removal
– Prevention of future obstructions in high-risk cats
Preoperative Considerations
– Confirm diagnosis and ensure the cat is stabilized (hydration, electrolytes, stabilization of
obstruction if present)
– Imaging (ultrasound, radiographs) to evaluate the urinary tract and stones
– Blood work (CBC, chemistry) to assess overall health
– Owner counseling regarding prognosis, potential complications, and post-op care
Postoperative Care:
– Pain management (NSAIDs, opioids as needed)
– Antibiotics if indicated
– Monitor for edema or bleeding
-Paper based litter and en E collar are must for desired outcome after the surgery.
– Feed a special urinary diet if recommended
– Observe for signs of urinary difficulty, bleeding, or swelling
– Follow-up appointments for suture removal and assessment

Potential Complications
– Urinary leakage or fistula formation or urine scalding of tissues.
– Stricture or narrowing of the new urethrostomy
– Infection or abscess formation
– Postoperative swelling or edema
– Urinary incontinence (rare)
– Recurrent obstruction (less common but possible)
Prognosis
– Generally good in experienced hands, especially for recurrent urethral obstruction
– Long-term success depends on proper surgical technique and postoperative care
– Most cats will require additional medical management or dietary modifications as underlying
causes still exist after surgery and need to be addressed long life long.
